The panel will receive two reports from the Office of Services Review regarding qualitative case reviews and case process reviews.

3. Fatality Reviews

Utah Code Section 62A-16-302 directs the panel to review Fatality Review Committee reports for individuals who had an open case for child welfare services within one year of death. The meeting will be closed for a portion of this agenda item in order for the panel to discuss individual child welfare cases. Following the closed portion of the meeting, the meeting will be reopened to the public for any further discussion and for any action by the panel.

• Motion to close the meeting to the public pursuant to Utah Code Subsection 52-4-205(2)(b) for the purpose of reviewing child fatality review reports.
