· Approval of the minutes of the October 13, 2015, and December 3, 2015, meetings

2. Qualitative Case Review and Case Process Review

The Office of Services Review will report the findings of their qualitative case review and their case process review of the Division of Child and Family Services. The panel will discuss trends and areas of concern identified by the Office of Services Review. The Division of Child and Family Services will advise what action has been taken, or should be taken, to address areas of concern.

3. Fatality Reviews

Utah Code Section 62A-16-302 directs the panel to review Fatality Review Committee Reports and Agency Responses for individuals who had an open case for child welfare services within one year of death. The meeting will be closed for a portion of this agenda item in order for the panel to discuss the reports and agency responses. Following the closed portion of the meeting, the meeting will be reopened to the public for any further discussion and action by the panel.
