The Utah's Legislative Fiscal Analyst facilitates an annual survey of executive and judicial salaries. A copy of the survey instrument for this activity is available below. 

If you are a member of the Western Legislative Fiscal Officers Association and have been notified by mail that we seek your input on the survey, please download a copy of the form, fill it out, and retun it via e-mail to gsyphus@utah.gov by August 10, 2007. 

The form can be read directly in Mircosoft Internet Explorer, or downloaded and accessed using Microsoft Excel or another spreadsheet application.
