To: Scott Jenkins,
Subject: A ten-minute research study from UNC-Chapel Hill
Date: Tue Apr 29 15:47:27 MDT 2014
My name is John Cluverius and I am a Ph.D candidate in the political science department at the University of North Carolina at Chapel Hill. I am conducting research on how state legislators across the country manage the many demands of the job. You experience as a legislator would be very valuable in this project. Would you or someone in your office be willing to complete a confidential survey on this topic? If so, please click the link below.
The survey will take about five minutes to complete. I am required by UNC’s Institutional Review Board (IRB) to keep your responses confidential. That means your responses and your decision to participate or not participate will never be disclosed to anyone. In addition, the data file with your responses will never contain your name or contact information (only an anonymous identification number) and will be securely encrypted and never distributed to anyone. This will prevent the possibility of deductive disclosure of your responses. If you have further questions, you may contact me at firstname.lastname@example.org or 919-962-0430, my advisor, Dr. Thomas M. Carsey, at email@example.com or 919-962-1207, or the IRB at IRB_subjects@unc.edu. Please reference study #14-420. Thank you very much for your help in this study.
Clicking on the link to the survey represents your agreement to participate. It means you have read the information provided above and asked all the questions that you have at this time. By clicking you voluntarily agree to participate in this research study.
