Senator D. Edgar Allen proposes the following amendments:
1. Page 1, Line 25: Delete "upon approval" and insert "approved"
2. Page 1, Line 26: After "Administration" insert "as of 1 July 1999"
3. Page 2, Line 28: After "education" insert "as provided or prescribed by a physician licensed under Title 58, Chapter 67, Utah Medical Practice Act, or Chapter 68, Utah Osteopathic Medical Practice Act"
4. Page 2, Line 41: After "podiatric appliances" delete the rest of the line and insert:
"prescribed by a physician licensed under Title 58, Chapter 67, Utah Medical Practice Act, or Chapter 68, Utah Osteopathic Medical Practice Act, or a podiatric physician licensed under Title 58, Chapter 5a, Podiatric Physician Licensing Act."
6. Page 2, Lines 42-43: Delete lines 42 and 43
