1. Page 5, Line 143: After Line 143 insert "(c) work within the FACT initiative to identify community partnerships to facilitate delivery of services based on family assessments;"
Renumber remaining subsections accordingly.
2. Page 5, Line 144: Delete "regularly" and after "review" insert "no less than twice each month"
