6 This act modifies the Driver Licensing Act by requiring that specific information from the
7 application for a driver license or identification card be forwarded to the Selective Service
8 System for registration as required by federal law. The act also specifies that refusal will
9 result in the denial of the license or card.
10 This act affects sections of Utah Code Annotated 1953 as follows:
11 AMENDS:

13 ENACTS:
14 53-3-234, Utah Code Annotated 1953
15 Be it enacted by the Legislature of the state of Utah:
16 Section 1. Section 53-3-234 is enacted to read:
17 53-3-234. Driver license application -- Selective Service Registration -- Statement.
18 (1) The following information for each male United States citizen or immigrant under the
19 age of 26 shall be electronically transmitted by the division to the Selective Service System:
20 (a) name;
21 (b) address;
22 (c) Social Security number; and
23 (d) date of birth.
24 (2) In acknowledgment, each application for any type of license to operate a motor vehicle
25 in this state, shall have the following statement included in the signature box:
26 "By submitting this application, I am consenting to registration with the Selective Service
27 System, if required by federal law."
28 (3) Refusal to consent to the release of information to the Selective Service System shall
29 result in the denial of the license.
30 Section 2. Section 53-3-804 is amended to read:
31 53-3-804. Application for identification card -- Required information.
32 (1) To apply for an identification card, the applicant shall appear in person at any license
33 examining station.
34 (2) The applicant shall provide the following information to the division:
35 (a) true and full legal name and address;
36 (b) date of birth as set forth in a certified copy of the applicant's birth certificate, or other
37 satisfactory evidence of birth, which shall be attached to the application;
38 (c) Social Security number or temporary identification number (ITIN) issued by the
39 Internal Revenue Service for a person who does not qualify for a Social Security number;
40 (d) place of birth;
41 (e) height and weight;
42 (f) color of eyes and hair;
43 (g) signature;
44 (h) photograph; and
45 (i) organ donor indication, if desired.
46 (3) The requirements of Section 53-3-234 shall also apply to this section for each person,
47 age 16 and older, applying for an identification card. Refusal to consent to the release of
48 information shall result in the denial of the identification card.
