2. Page 7, Lines 191 through 194 :
191 antifreeze solution by volume with which the system is filled,

the date the antifreeze was
192 replaced, if applicable, } the name of the contractor that tested

the antifreeze solution,
193 the contractor's license number,

a statement indicating if the entire system was drained and
194 replaced with antifreeze, } and a warning to test the concentration of the antifreeze solutions at
